If you need a copy of your records or would like your records transferred to another provider, please note that a completed authorization form is required before records can be released or transferred. Patients with questions regarding their account should also refer to the website for additional information and updates. Account reviews are underway, and any applicable refunds will be calculated in accordance with the terms of each patient’s treatment agreement.
